Yes, this is a fairly long form but it is very difficult to give an accurate estimate without seeing the watch so the more info we get from you the more accurate and complete your estimate will be.  Please also supply your phone number and a good time we can contact you; this will make it much easier for us to contact you if we have any questions and to calculate the best possible estimate.
